Age : 33 - Gender : Male

Diagnosis :

Alveolar hydatid cyst disease.

Findings :

On T2 axial images, a large hypointense mass with geographic shape in the hepatic dome is seen. On post-contrast series, there is only a small amount of peripheral enhancement of the mass, no significant enhancement or diffusion restriction is present.

History :

A patient with incidentally discovered hepatic mass.

İs it giant sclerosing hepatic hemangioma???

Irreguler contour plus non-enhancing pattern firstly made me consider on cholangiocarcinoma but we expect to see its fibrotic component enhancing on late phase which is not exist in this case. On T2w images, little tiny hiperintense focis and large hipointense areas may be result of tiny cysts and fibrotic component of E. alveolaris in order and combining these with the first findings, echinococcus is counted for differential diagnosis.

Patient is 33 years old...and there is no dilated intrahepatic byle ducts...i think its not a cholangioCA..

Hydatic Cyst... fibrolamellar carcinoma

This mass may be focal fatty area.

Maybe its a hepatic alveolar echinococcosis...
