Age : 21 - Gender : Female

Diagnosis :

Focal nodular hyperplasia

Findings :

The lesion is isointense relative to normal liver tissue on T2W and precontrast images. Postcontrast images demonstrate early arterial enhacement. On delayed phase images, once again it becomes indistinguishable from liver parenchyma . The central scar demonstrates high signal intensity on T2WI and enhancement on delayed phase image.

History :

A patient with right upper quadrant pain presented to the hospital.

Findings: A large mass right liver lobe, subcapsular location, arterial enhancement and hypervacularity, no wash-out on delayed phase, central scar, delayed enhancemet on scar. Diagnosis is FNH

I agree with Mr. Danaci, the case shows typical features of FNH. Lesion is isointense at precontrast T1 and T2-W images. Arterial enhancement, central scar showing enhancement on delayed phase are also characteristic findings of FNH.

The lesion have a central scar which can be compatible with FNH or fibrolamellar carcinoma. The central scar has high signal on T2WI and shows significant enhancement on delayed phase, typical for FNH.

Orange slice with central stellate scar ... i think it is FNH !!!
