A diffuse HCC developed on the basis of the cirrhosis is the most likely diagnosis. There are also gall bladder calculi and ascites due to cirrhosis.

there is a mass in gallbladder fossa extending to the liver parenchyma. Mass shows infiltrative pattern in the liver. Also there are retractions of liver surface and other small sized lesions in the liver. I see portal lymph nodes with pathologic size and shape. Peritoneal carsinomatosis and ascites is also noted. My first diagnosis is galbladder adenocarcinoma.

Most likely diagnosis is the gallblader adenocarcinoma with liver invasion. Liver metastasis, peritonitis carcinomatosa , periportal- aortacaval metastatic lymph nodes .
